The tables currently used with the LIMRI system should work with little modification.
InventoryList

IID - Inventory ID. This will be an arbitrary number identifying the inventory

PIN - Instead of a 4-digit PIN, this will be the user's ID number.

Voice - This will be either the role of the user or a supervisory user (like a counsellor in the current system).

For supervisory users, a simple query to the database could get all inventories for which that user is the supervisor.

 SELECT IID FROM InventoryList WHERE PIN='$idnumber' AND Voice='C';

Assignments

IID - Inventory ID.

SID - Sheet ID. Different sheets can be made with different topics. These may be assigned based on status (faculty, staff, student, etc.).

Voice - This will always be the user taking the assessment/training.

Status - U/I/C - U seems to be no progress, I is incomplete, and C is complete.

Completed - This is the date the sheet was completed.

Answers

IID - Inventory ID.

QID - Question ID.

Voice - This will always be the user taking the assessment/training.

Source - This is probably irrelevant for this assessment/training, but could be the same as Voice if needed.

Target - This was empty for the current tables, and is probably irrelevant for this assessment/training.

Response - This is the user's response.

Timestamp - This is the time of the user's response.
